Axial: T2, FLAIR, Mag, phase, susceptibility, DWI, ADC. Time-of-flight volumetric MRA\n\nComparison: CT brain (18/06/2020 and angiogram head 19/06/2020)\n\nFindings:\nWithin the posterior right insular cortex there is a region of diffusion restriction which demonstrates associated high T2/FLAIR signal. There are several smaller areas, 5 mm and under, of diffusion restriction seen within the right occipital and post the temporal lobes. There is mild restricted effusion within the left side of the splenium of the corpus callosum and the punctate focus of diffusion restriction within the posterior aspect of the left para into conchal gyrus. No areas of susceptibility artefact are demonstrated to suggest haemorrhagic transformation.\n\nThere is extensive T2/FLAIR hyperintensity, likely secondary to chronic small vessel disease.\nNo hydrocephalus.\n\nMRA:\nCompared to a previous angiogram, the distal right M1 occlusion has resolved. There no aneurysms, high-grade stenoses or large vessel cutoff seen within the circle of Willis or its major branches. The left posterior communicating artery is noted to be very small.\n\nImpression:\nSeveral, small right MCA territory infarcts, the largest affecting the right insular cortex. Small infarcts affecting the posterior aspect of the left parahippocampal gyrus and left side of the splenium of the corpus callosum. No evidence of haemorrhagic transformation.
